Amy Goldsmith
COO

With over 23 years of dedicated service in the Post-Acute skilled nursing sector, Amy Goldsmith, MA, CCC-SLP; RAC-CT, brings a wealth of experience to the field. In January of 2023, she stepped into the position of Chief Operations Officer at R2, spearheading the effort to unearth reimbursement opportunities in the complex realm of skilled nursing care.

Amy has an impressive track record in team development and process enhancement across various payer segments, consistently adapting to the unique requirements of facility teams. Throughout her career, much of which has been spent in operations, she notably served as the Senior Vice President of Clinical Reimbursement for Sava Senior Care. In this capacity, she championed the creation of an in-house case management team and the establishment of education and reimbursement specialists, benefiting over 200 facilities spanning 24 states.

Amy’s steadfast dedication to supporting the elderly, combined with her aptitude for fostering productive collaboration within diverse teams, has enabled her to guide thriving Assessment Coordination teams, Rehabilitation groups, and Care Coordination nurses. Her path to excellence encompasses the attainment of her RAC-CT certification in 2008, followed by her CMAC in 2018. Amy also holds a license as a Speech and Language Pathologist, a credential she acquired in 1998 following her master’s degree completion at the University of Maine.

In recognition of her dedication to elevating patient care, Amy was recently appointed to the AAPACN Education Foundation Board and the CMS MEDCAC patient advocacy council. In these roles, she tirelessly advocates for the needs of both patients and the dedicated nurses who provide care in this often challenging industry.

Sally Fecto
VP of Quality

Sally has extensive experience in LTACH, IRF, acute care, and skilled nursing facilities since 1982. Her career has progressed from CNA to staff nurse, unit manager, and MDS coordinator at the facility level.

Over the past 20 years, Sally has held various management roles, including Regional Clinical Reimbursement Specialist, Director of Clinical Reimbursement, VP of Operations, and VP of Field Operations for a healthcare consulting group. She has traveled nationwide, providing teaching, training, audits, and consultations to assist providers in all areas of clinical reimbursement and quality.

Sally joined Zimmet Health Services Group in 2019 and transitioned to her current position as VP of Clinical Reimbursement and Consulting Services at Reimbursement Reimagined in 2023. She has helped facility and consulting teams identify strategies and solutions for challenges in skilled nursing facilities, particularly in audits, reimbursement, quality, skilled documentation, and Quality Measures, achieving positive outcomes.

Deborah Fein
VP Education & Training

Debbie is a dedicated Gerontologic RN with over 30 years of experience in skilled nursing. She has excelled in various roles within SNFs, as well as in consulting and leadership positions. Debbie specializes in guiding teams to achieve their goals by sharing her extensive knowledge of the RAI process and Medicare, Managed Care, and Medicaid reimbursement systems, always prioritizing quality outcomes. Her mission is to support every team’s success through education, respect, and teamwork.

Karen Lightner
VP MDS Support Services

Karen Lightner brings 30 years of experience in post-acute care nursing, including roles such as SNF charge nurse, supervisor, home care provider, MDS coordinator, and clinical consultant. She spent 22 years as VP of Consulting Services with Zimmet Healthcare Services Group. In January 2023, she joined R2, Zimmet’s sister company, as VP of Remote MDS Support, leading a team of reimbursement and MDS specialists.

Karen is certified in Gerontological Nursing and has held the RAC-CT credential since 2015. She is also a member of AAPACN and PAANAC.

Katy O’Connor
M.S.P.T, VP Case Management Services

Katy is a licensed physical therapist with over 20 years of experience in the post-acute clinical arena. Her career as an operational leader has included roles such as practitioner, regional manager, consultant, vice president of clinical reimbursement, and chief reimbursement officer. Katy has focused on driving performance improvements across skilled nursing facilities at every level, from patient care to corporate operations.

She earned her Master of Science in Physical Therapy from the University of Colorado Health Science Center in 2001.

Sara Dettore
VP of Remote Medicare and Medicaid Monitoring Services

With over 23 years of post-acute care experience, Sara Dettore, MBA; OTR/L; LNHA; RAC-CT, brings a breadth of clinical and operational knowledge to the rapidly changing reimbursement landscape.  Sara has extensive experience with PDPM processes and reimbursement and has been leading teams to PDPM success since 2019.

Sara has varied experience as a clinician, operator, and leader, making her uniquely skilled to command a team reimbursement specialists.  Her experience as Vice President of Operations and Assistant Vice President of Nursing Home Clinical Reimbursement at Archcare garnered her the experience and acumen to create powerful interdisciplinary teams who were able to operationalize reimbursement data into actionable plans for their residents and facilities.

Sara has spent her entire career working in post-acute care and has expertise in navigating the healthcare landscape for our country’s aging and elderly.  Sara’s unique background makes her a skilled facilitator in any facility setting or dynamic where she is always able to effect positive change.
